Flu shots available at Tilghman Health Clinic

If the patient is over 18 years of age, they can visit the clinic during normal operating hours. The staff will have them complete the appropriate paperwork and administer the vaccine. If the patient is under the age of 18, the clinic must have the consent of the parent/guardian before being able to administer the flu vaccine.

There are three documents that the parent/guardian of a minor would need to fill out and send in with the student to get the flu vaccine administered within the clinic. Again, these forms only apply to patient under age 18. Our clinic will provide the appropriate paperwork onsite for anyone over 18 years of age.

2) VaxCare Consent Form (under 18)
3) Influenza Vaccine Questionnaire

The clinic requires copies of insurance cards on all patients who receive service. For patients that use insurance, the flu vaccine visit will be submitted to their insurance company for payment. For any outstanding balance after insurance pays, the patient/guardian would be responsible for the remaining balance. For patients that do not have insurance or elect not to use it, our billing office would send them a bill in the mail for the services rendered within our facilities.
